Megan Fox has stirred up strong reactions online. That’s because of a video in which many reacted to one major detail. Was it really Megan Fox, or had she ‘completely altered’ her looks?

Megan Fox has sparked a wave of reactions online after appearing in a new video that has left some fans struggling to recognize the Hollywood star.

The Transformers actress, 40, appeared alongside director Karyn Kusama in a Letterboxd video, where the pair discussed their favorite movies. The clip has been widely shared on social media, but instead of focusing on the films, many viewers focused on Fox’s appearance.

Some claimed she looked very different from the woman they remembered, while others questioned her hair and speculated about cosmetic procedures.

“What happened to the Megan Fox we all knew? My God, she ruined herself,” one person wrote.

“Somebody did her way wrong, and she has a bad wig,” another person added.

A third person took it even further, saying, “The plastic surgery industry owes women an apology.”

Megan Fox leaves fans horrified with appearance
“She’s going to be different than she did almost 20 years ago. She was gorgeous, but it’s the plastic surgery that is making her age quite horribly,” a fourth wrote.

However, there is an important detail about the footage of Megan Fox to remember.

Although Letterboxd shared the video in October 2026, it was actually filmed in October 2025, when Fox and Kusama reunited for a special screening of Jennifer’s Body in Los Angeles.

At the time, Fox was only months into motherhood, having given birth to her fourth child. She welcomed her daughter, Saga Blade, in March 2025.

Fox has previously spoken openly about how difficult the period after giving birth was for her. Around that time, she described experiencing brain fog and struggling with a lack of sleep.

Spoke openly about cosmetic procedures
Despite this, much of the online discussion focused on her appearance, with some users making harsh comments about her face and hair.

This is not the first time Fox has faced questions about her changing appearance. The actress has previously discussed cosmetic procedures she has undergone, including breast augmentation and rhinoplasty. She has also spoken about Botox and fillers.

However, she has pushed back against speculation about procedures she says she has never had. Fox has also criticized the way women are judged for their appearance and the stigma surrounding cosmetic surgery.

“I feel like there’s this stigma, and I’m not going to win,” Fox said on the popular podcast Call Her Daddy in 2024. “I hope it sets some people free.”

The latest video has once again sparked debate online, with some viewers criticizing Fox while others have questioned why so much attention is being paid to her appearance.
