Viral Dwayne Johnson Photo Sparks Health Concerns — But the Image Was Edited

A viral photo of Dwayne Johnson has sparked concern among fans after appearing to show the Hollywood star with a dramatically smaller physique. However, the image circulating online does not accurately represent how the actor looked at the event depicted.

The picture spread rapidly on X, where users questioned the 54-year-old star’s appearance and wondered whether something had happened to his health. The post attracted more than four million views, with reactions ranging from jokes about his reduced size to genuine expressions of concern.

One user remarked that Johnson’s shoulders once seemed to need their own ZIP code, while another joked that he had gone from “The Rock” to “The Pebble.” Other commenters were less amused, with some suggesting he looked unwell and others hoping he was doing okay.

But the viral image was not an authentic representation of Johnson’s appearance.

The Truth Behind the Viral Photo

The picture was altered from an older photograph taken at CinemaCon in Las Vegas on April 11, 2024. Johnson attended a Walt Disney Studios presentation at the event, wearing a white tank top.

According to a Community Note attached to the viral post, the image had been edited using artificial intelligence. The original photograph showed Johnson with a considerably more muscular build than the version shared online.

As a result, the picture that prompted so much speculation should not be treated as evidence of a sudden change in his health or physique.

However, Johnson has genuinely undergone a significant physical transformation recently — for an entirely different reason.

Why Dwayne Johnson Really Looks Different

On October 20, Johnson was photographed on the set of Lizard Music in Coney Island, Brooklyn. In the images, he looked almost unrecognizable, sporting a gray beard, a bucket hat and a coat as he transformed into a character known as the Chicken Man.

The character is described as a man in his seventies, requiring Johnson to undergo a substantial makeup transformation. The prosthetics were created by Kazu Hiro, the Oscar-winning makeup artist known for transforming Gary Oldman into Winston Churchill and for his work on The Smashing Machine.

Johnson revealed in an August interview with Deadline that the makeup process takes around five hours each day.

The transformation is part of his commitment to portraying the unusual character convincingly, rather than evidence of the unexplained physical change suggested by the doctored viral photo.

He Previously Revealed Plans to Lose Muscle

Johnson has openly discussed changing his physique for the Chicken Man role.

During the 2025 Toronto International Film Festival, he reportedly told the audience that he had lost around 30 pounds of muscle and intended to slim down further for the part.

This is not the first time the actor has altered his famously muscular appearance for a movie. He also changed his physique for The Smashing Machine, in which he portrayed pioneering mixed martial artist Mark Kerr.

Johnson’s recent transformation reflects the demands of his acting work, with the star taking on roles that require him to move away from the powerful physical image audiences have come to associate with him.

What Is Lizard Music About?

The upcoming film is based on Daniel Pinkwater’s 1976 children’s novel of the same name.

Its story follows a boy who discovers strange lizard musicians while watching late-night television. His unusual discovery leads him to meet an eccentric man known as the Chicken Man, who has an elderly chicken.

Benny Safdie, who directed The Smashing Machine, is directing the film. Theo Clark Leber and Tim Heidecker are among the cast members.

Johnson has previously described how quickly he connected with the project. After hearing Safdie’s pitch, the actor reportedly needed only about 45 minutes to decide he wanted the role, telling the filmmaker, “I am your Chicken Man.”

Lizard Music is scheduled to arrive in theaters on Christmas Day 2027 through Amazon MGM Studios.

While the altered image prompted widespread speculation, the available information points to an edited photograph rather than a genuine depiction of Johnson at the event. His real-life transformation, meanwhile, is connected to the demanding character he is preparing to bring to the screen.
